IRS Form 1095-C provides information about the health insurance coverage Edgewood College offers and your election to, or not to, participate. Because our group health plans are fully insured, you will also receive a Form 1095-B from Group Health Cooperative. Like Form 1095-C, Form 1095-B provides enrollment information about you and, if applicable, your dependent’s health coverage.  You should retain both forms with your 2020 tax records.

If you consented to receive Form 1095-C electronically, it is now available.  To access your electronic form and the Instructions for Recipient, follow the instructions found here:  Electronic Tax Form 1095-C Retrieval.  

If you did not consent to receive Form 1095-C electronically:

• It has been mailed to your address on record.
• Consider doing so now – the form will be immediately available to you.  Instructions are found here:  Consent Instructions.
