FLU VACCINE CLINIC

Available to Faculty, Staff, and family members who are at least 6 years of age
The Clinic is provided by VaxPro Services (A wellness company focused on providing on-site flu vaccine clinics)
VaxPro direct bills Dean Health Plan for the vaccine as preventative healthcare.
They also direct bill other carriers, including: Anthem/Blue Cross Blue Shield, Arise Health Plan, Health Partners, Humana, GHC, Medicare Part B, Prevea360, Quartz, UMR, United Healthcare, WEA Trust, and WPS. If you have coverage with one of these other providers, you are welcome to sign up for your vaccine at our October 14th Clinic.
Uninsured or non-covered employees and family members may receive the vaccine at a cost of $34 per shot for a quadrivalent flu vaccination. Fees are collected at the flu clinic appointment
WHAT YOU MUST DO
- Register for your flu vaccine clinic appointment on VaxPro’s online scheduler or using the QR code via this link - 2021 Flu Vaccine Clinic QR Code
- You will complete screening questions and attach your insurance card when registering for your flu vaccine clinic appointment
- Read the Vaccine Information Statement before the Clinic
COVID-19 PRECAUTIONS REQUIRED
- Wear a mask that snugly covers your mouth and nose at all times while at the Clinic
- Your temperature will be taken as part of the check-in screening process
- Maintain social distancing of at least 6 feet while approaching and leaving the Clinic
- Avoid arriving early to your appointment or staying afterward
- If you or a family member planning to attend the flu clinic have any COVID-19 symptoms, or have had close contact with someone who tested positive for COVID-19 in the past 2 weeks, please do not attend
